The Montana living will form is a legal document that is utilized by an individual (Principal) who would like to prepare in advance, while they are of sound mind, exactly how they would like to inform family, clergy, medical providers etc..to provide for their medical care when they are no longer able to care for themselves or make their own medical decisions. If the Principal decides to appoint a health care agent, who would make their decisions when they are no longer able, then the Principal should provide a copy of this document to their agent so that they are prepared to properly honor the Principal’s decisions for themselves. This document may be changed or revoked at any time

How to Write
Step 1 – The Principal must download the declaration and enter the date of execution of the document:
- Enter the name of the Principal into the first paragraph
- The Principal must carefully read and agree to the following three paragraphs
- If any of the statements in the paragraphs do not apply, the Principal may strike them by drawing a line through any of the statements in the document
- If the Principal would like to provide additional information, they may add a sheet to the document
Step 2 – Signatures/Witnesses –
- The Principal must read and agree to the statement located in the center of the page
- If in agreement, the Principal must enter their signature
- Date the signature
- Provide the City, County and State of Residence
- Submit the Principal’s date of birth in mm/dd/yyyy format
- Enter the Principal’s Social Security Number
Witnesses – Each witness must read and agree to the statement provided. Submit:
- The date of the witness signatures in dd/m/yy format
- Each witness must provide their signature
- Each witness must provide their address
