Nevada Last Will and Testament Form

The Nevada last will and testament is a legal document that may be utilized to guide any person (Testator) to personally  create a document that will reflect their wishes pertaining to how they would like to have their estate distributed by the Testator’s executor after the death of the testator, among their beneficiaries. An executor may be established within this document. The document may only be completed by the Testator while they are of sound mind. The Testator may also revoke or change the document at their discretion.

How to Write

Step 1 – Provide the individual’s name at the top of the page in the line provided to establish the Testator – Proceed:

  • Enter the Testator’s name
  • City and County of residence
  • Review “Expenses and Taxes” title

Step 2 – Provide an Executor – The Testator may establish an executor by providing the following information:

  • The name of the executor
  • Address
  • County of residence
  • State of residence

In case the first selected executor is unable to serve, an alternate may be provided. Enter :

  • Submit the name of the alternate
  • Address
  • County
  • State

Step 3 – Disposition of Property –Enter all required beneficiary information, if there are more beneficiaries than will fit onto this form, add a sheet with the proper information and attach it to the document – Submit:

  • Each Beneficiary’s name
  • Address
  • Relationship to the Testator
  • Submit the last four (4) digits of the beneficiary’s Social Security Number
  • The Testator must read over and agree to the next paragraphs
  • If there will be additional information, the Testator may add a separate sheet and attach

Step 4 – Titled Sections – Review:

  • Omissions
  • Bond
  • Discretionary Powers of Personal Representative (subsections A through K)
  • Contesting Beneficiary
  • Guardian Ad Litem Not Required
  • Gender
  • Assignment
  • Governing Law

Step 5 – Binding Agreement – The Principal must read and agree to the first paragraph. Then enter the following:

  • Testator’s name
  • Date the execution of the document(dd/mm/yyyy)
  • Testator’s name
  • Printed name
  • Date of signature in dd/m/yy format
  • Testator’s name

Step 6 – Witness Signatures and Notary Public – Signatories must be present before one another and before the witness of a notary

Witnesses –

  • Witnesses must enter their respective signatures
  • Current addresses

Testamentary Affidavit –

  • The notary shall acknowledge this document once completed
  • The notary will provide the names of all signatories
  • Testator will provide their signatures
  • Witnesses shall enter signatures (respective)
