Washington Last Will and Testament Form

The Washington last will and testament is a legal document that is provided for a person (Testator) so that they may specify exactly how they would like to have their estate divided among their beneficiaries. The estate could consist of all or any of the following: personal property, real property and or all financial accounts, (banking, retirement etc.). The estate would be distributed by an executor (representative) selected by the testator and will be distributed once the testator is deceased.

How to Write

Step 1 – Enter the Testator’s name at the top of the page to establish the document, then enter into the paragraph following:

  • Testator’s name
  • City of residence
  • County of residence
  • Testator must read “Expenses and Taxes”

Step 2 – Select an Executor – The Testator may establish their executor by providing the following information:

  • The name of the selected executor
  • Address and County of residence
  • State of residence

If the primary appointed executor is unable to serve, the testator may appoint an alternate by provision of the following information:

  • Submit the name of the alternate agent
  • Enter their Address
  • County of residence
  • State of residence

Step 3 – Disposition of Property – In order to properly dispose of the estate provide the following information:

  • All Beneficiary’s names
  • Beneficiaries addresses
  • Relationships to testator
  • Provide the last four digits of each beneficiary’s Social Security Number
  • Read the paragraphs provided in the section
  • If there are more instructions, a separate sheet may be added and attached to the form

Step 4 – Titled Sections – Review:

  • Omissions
  • Bond
  • Discretionary Powers of Personal Representative (subsections A through K)
  • Contesting Beneficiary
  • Guardian Ad Litem Not Required
  • Gender
  • Assignment
  • Governing Law

Step 5 – Binding Agreement – Read and agree to the first paragraph. Enter the following:

  • Testator’s name
  • Submit document’s date in mm/dd/yyyy format
  • Testator’s signature
  • Testator’s printed name
  • Date of signature in dd/m/yy format
  • Testator’s name

Step 6 – Witness Signatures and Notarization – Signatories must provide signature before a notary public

Witnesses –

  • Witnesses signatures
  • Addresses

Testamentary Affidavit –

  • Once the document has been completed, it must be witnessed by a notary
  • The notary will enter the names of all signatories
  • Testator’s signature
  • Witnesses signatures
