Michigan Advance Health Care Directive (Living Will) Form

The Michigan advance health care directive(living will) is a legal document that will allow an individual (Principal) to create a document, while they are of sound mind that will declare to medical providers as well as the Principal’s family, their specific end of life wishes. The document will legally protect the Principal’s decisions when they are no longer able to communicate with medical providers on their own. It will be a written record that will carry the weight of the Principal’s decisions. As long as the Principal has any ability at all of communicating that the form no longer reflects their wishes, the document may be changed or revoked any time.

How to Write

Step 1 – Download the document and establish the Principal of the document by entering their name into the first line of the declaration form

  • Next,the Principal must carefully review the next paragraph
  • The Principal must then submit, into the lines provided, a description of their desires regarding their medical treatment (if more space is required, add a sheet continuing the description and attach it to the form.
  • The Principal must read and agree to the remaining paragraphs prior to signing the form

Step 2 – Signatures and Witness –

  • Submit the date of the Principal’s signature (mm/dd/yyyy)
  • The Principal must submit their signature
  • Principal’s address

Witnesses – The witnesses to this document must read and consider the witness statement. If in agreement, each witness must submit the following:

  • Witnesses Printed names
  • Witnesses Signatures
  • Witnesses Complete Addresses
