Kentucky Last Will and Testament Form

The Kentucky last will and testament is a legal document made available for individuals who would like to use a standard will to provide instructions pertaining to how they would prefer to have their estate distributed to beneficiaries, after the Testator’s death. The document will provide the availability to enter information to appoint a trusted person as executor. Once complete, the document will require two witnesses.

How to Write

Step 1 – Once the document is downloaded, enter the Testator’s name in the first line of the form.  The Testator would then submit the following:

  • The Testator’s full name
  • City
  • County
  • Review all of the titled section “Expenses and Taxes”

Step 2 – Appoint an Executor – The Testator may appoint an executor by submission of  the following information:

  • The appointed executor’s name
  • Address
  • County
  • State

The Testator may appoint an alternate in advance, by providing the following:

  • Alernate’s full name
  • Address
  • County
  • State

Step 3 – Disposition of Property –The Testator must provide all required information about their beneficiaries. If there are more than three, add them on a separate sheet with all of the same information and attach:

  • Name of each respective beneficiary
  • Current address
  • Relationship
  • Provide the required last four (4) digits of the beneficiary’s Social Security Number (SSN)
  • The Testator must read the next paragraphs
  • If additional instructions are required, the Testator may provide them on a second sheet and attach to the form

Step 4 – Titled Sections – The Testator must review:

  • Omissions
  • Bond
  • Discretionary Powers of Personal Representative (subsections A – K)
  • Contesting Beneficiary
  • Guardian Ad Litem Not Required
  • Gender
  • Assignment
  • Governing Law

Step 5 – Binding Agreement – The Testator must read and agree to the first paragraph. Submit:

  • Name of the Testator
  • Enter the date of the execution of the document in dd/m/yy format
  • Testator’s name
  • Printed name
  • Submit the date of the Testator’s signature dd/m/yy
  • Testator’s name

Step 6 – Witness Signatures

Witnesses –

  • Witnesses must provide signatures (respective)
  • Provide Current addresses

Testamentary Affidavit (Optional) –

  • The notary shall acknowledge this document after completion
  • The notary must provide the names of all parties in the lines provided
  • Testator must provide their signature
  • Witnesses shall provide signatures (respectively)
