The Illinois last will and testament is a document that has been created to assist a Testator by providing instruction regarding how to state how they would like to have any of their estate (personal, real property or financial accounts) distributed upon their death, to their listed beneficiaries. The document will organize the process of creating a proper will and testament.

How to Write
Step 1 – To establish the author of the document, submit the full name of the Testator in the first line on the first page. In the following paragraph, enter:
- The Testator’s name
- City of residence
- County of residence
- Read the remaining paragraphs
- Review the title “Expenses and Taxes”
Step 2 – Personal Representative – The Testator shall appoint a personal representative by provision of the following:
- Submit the name of the initial delegated representative
- Address
- County
- State
Should the first delegated representative become unable or unwilling to serve, an alternate may be delegated in by submitting the following information:
- The name of the aleternate representative
- Complete address
- County of residence
- State
Step 3 – Disposition of Property –Begin by entering all required beneficiary information. If the Testator will provide for more than three beneficiaries, add an additional sheet with all of the same information and attach the sheet to this form :
- Full name of all beneficiaries
- Current addresses
- Relationships
- Submit the last four (4) digits of the respective beneficiary’s Social Security Number (SSN)
- The Testator must carefully read through the next paragraphs
- If additional instructions would apply,theTestator should add them on a separate sheet
Step 4 – Titled Sections and all Subsections- The Testator must review the following:
- Omissions
- Bond
- Discretionary Powers of Personal Representative (subsections A – K)
- Contesting Beneficiary
- Guardian Ad Litem Not Required
- Gender
- Assignment
- Governing Law
Step 5 – Binding Agreement – The Testator must read and agree to the first paragraph then enter the following into the second paragraph:
- Testator’s name
- The date of the exectution of the document in dd/m/yy format
- Testator’s full name
- Printed name
- Submit the date of the signature in dd/m/yy format
- Testator’s name
Step 6 – Witness Signatures
Witnesses –
- Witnesses must submit their respective signatures
- Current addresses
Testamentary Affidavit (Optional) –
- Once completed, the notary shall acknowledge this document after completion
- The notary shall complete the remaining section
- The Testator must submit their signature
- Witnesses must enter their signatures
