Wyoming Last Will and Testament Form

The Wyoming last will and testament is a legal document that provides guidance to any individual (Testator) who would care to create a written document, a standard will, so that the testator will be able to provide specific instructions prepared in the event the testator passes away. The document will provide the ability to name an executor to distribute the estate. This estate may include personal property, real property and financial accounts. The executor will be responsible to distribute the testator’s estate honorably and according to the written wishes of the Testator. As well, the document will allow the Testator to enter beneficiary information and name any property to be bequeathed to each.


This document must be completed by the Testator while they are of sound mind. The document may be changed or revoked also at the wishes of the principal.

How to Write

Step 1 – The principal must download the document – Submit the author/testator’s name at the top of the page – Proceed:

  • Submit the Testator’s name
  • City of residence
  • County of residence
  • Review “Expenses and Taxes”

Step 2 – Assign an Executor – The Testator may choose to  establish an executor by entering the following:

  • The Executor’s full name
  • Address
  • County of residence
  • State of residence

If the primary Executor is, at any point, unreasonably unavailable or unwilling to continue service, an alternate may be appointed in advance. Provide the following:

  • The alternate agent’s full name
  • Address
  • County and State of residence

Step 3 – Property Disposition –Enter all beneficiary information:

  • Beneficiary’s name(s)
  • Address(es)
  • Relationship(s)
  • Last four (4) digits of beneficiary’s Social Security Number
  • Testator must read the remaining paragraphs
  • If there are additional instructions, the Testator may add a separate sheet and attach it to this form

Step 4 – Titled Sections –

  • Omissions
  • Bond
  • Discretionary Powers of Personal Representative (subsections A through K)
  • Contesting Beneficiary
  • Guardian Ad Litem Not Required
  • Gender
  • Assignment
  • Governing Law

Step 5 – Binding Agreement – The Principal must read and agree to the first paragraph. Proceed by provision of the following information:

  • The Testator’s name
  • The date of the document’s execution in mm/dd/yyyy format
  • Testator’s signature
  • Principal’s printed name
  • Date of signature (dd/m/yy format)
  • The Testator’s name

Step 6 – Witness Signatures and Notary Public – Signatories must be present before a notary

Witnesses –

  • Witnesses respective signatures
  • Witnesses Addresses

Testamentary Affidavit –

  • The notary shall acknowledge this document
  • The notary will provide all required information
  • Testator’s signature
  • Witnesses signatures
