The Rhode Island last will and testament is a document that would be utilized by an individual (Testator) who would like to make advanced preparations, while they are of sound mind, as to how they would choose to divide their estate among their chosen beneficiaries, after the Testator’s death. Should the Testator choose, they may appoint an executor also by the use of this document. Beneficiaries may be listed along with the bequeathed property. The document will require two witnesses and, though optional, notarization. The document may be revoked or changed at any time.

How to Write
Step 1 – After the form has been downloaded – Enter the Testator’s name at the top of the page:
- Submit the Testator’s name
- City
- County
- Review “Expenses and Taxes”
Step 2 – Appointment of Executor – The Testator may establish an executor of their choice, by providing the following:
- The name of the executor
- Address (C0mplete)
- County
- State
If the first selected executor is unavailable, an alternate may be established – Enter the following:
- Alternate Agent’s full name
- Address
- County and State of residence
Step 3 – Disposition of Property –Submit required beneficiary information (required):
- Beneficiaries names
- Addresses
- Relationships
- Submit the last four (4) digits of the beneficiaries SSN’s
- Read the next paragraphs
- If additional instructions would be applicable, the Testator may add a separate sheet and attach it
Step 4 – Titled Sections –
- Omissions
- Bond
- Discretionary Powers of Personal Representative (subsections A through K)
- Contesting Beneficiary
- Guardian Ad Litem Not Required
- Gender
- Assignment
- Governing Law
Step 5 – Binding Agreement – The Principal must agree to the first paragraph. Proceed and submit:
- The Testator’s name
- Date of the document (dd/mm/yyyy)
- Testator’s signature
- Testator’s printed name
- Date of signature in dd/m/yy format
- Testator’s name
Step 6 – Witness Signatures and Notary Public – Signatories will be present before one another and a notary public

Testamentary Affidavit –
- The notary shall acknowledge this document by completing the notary section of the form
- Testator will sign before the notary
- Witnesses shall also provide signatures
