The Ohio last will and testament is a legal document (a standard will) that may be completed by any person (Testator) who would like to prepare their estate for distribution after the testator’s death. The document will guide the testator through the process of naming an executor as well, recording the information needed of the beneficiaries. Only the Testator may complete the form (or it must be done in the presence of the Testator) and it must be completed while the testator is of sound mind. The testator may also revoke the document at their discretion.

How to Write
Step 1 – Download the document – Enter the name of the Testator at the top of the page. Proceed by providing:
- The Testator’s name
- City
- County
- Read and review “Expenses and Taxes”
Step 2 – Naming an Executor – Establish an executor by submitting the following:
- Executor’s name
- Full address
- County of residence
- State of residence
In the event the initial executor will no longer have the ability to serve, enter the following information to elect an alternate agent (Optional):
- The full name of the alternate
- Address
- County and State of residence
Step 3 – The Distribution of Property –Provide the required beneficiary information:
- Beneficiary’s name
- Address
- Relationship to the testator
- Submit the last four (4) digits of the beneficiary’s Social Security Number (required)
- The Testator must read the next paragraphs
- Should the testator wish to provide additional beneficiaries information, enter it on a separate sheet, sign and date the additional sheet and attach
Step 4 – Titled Sections and Subsections
- Omissions
- Bond
- Discretionary Powers of Personal Representative (subsections A through K)
- Contesting Beneficiary
- Guardian Ad Litem Not Required
- Gender
- Assignment
- Governing Law
Step 5 – Binding Agreement – The Testator must review and agree to the first paragraph. Then enter the following:
- Testator’s name
- Submit the date of the document’s exectution (dd/mm/yyyy)
- Testator’s name
- Printed name
- Date of signature in dd/m/yy format
- Testator’s name
Step 6 – Witness Signatures and Notary Public – All signatures must be before a notary:
Witnesses –
- Witnesses signatures (respectively)
- Current witness addresses
Testamentary Affidavit –
- The notary shall acknowledge the document
- The notary will provide all signatory information
- The Testator will must provide signature
- Witnesses signatures
