The Florida living will is a legal document that is designed to allow any individual, while they are of sound mind, an opportunity to place into writing, their specific desires with regard to how their medical decisions are handled when/if they find they are facing an end of life scenario or in the event they are no longer able to communicate their decisions on their own as before. The Principal will, not only have the power to choose how they would like their medical care to be carried out (and should expect their medical team to implement their care as such) but they will have the opportunity to select and delegate an attorney in fact/agent who they trust, who will carefully oversee the Principal’s written desires with their end of life medical choices and work with their medical team to ensure that their last wishes are properly honored. This document will require the signatures and information of two (2) witnesses in order that it may be effective. This document may be changed by the Principal only, or it may be revoked at their discretion.

How to Write
Step 1 – Download the document – Begin by:
- Dating the document in dd/mm/yyy format
- Submit the Principal’s name
- Initial one of the statements that apply to the current medical condition of the Principal
- Carefully review the remaining three paragraphs- If any of the statements made are not the true wishes of the Principal, they may strike through those sentences to void those statements
Step 2 – Delegation of Attorney in Fact/Agent/Surrogate – Establish the surrogate by entering the agent’s following information:
- Full name
- Street Address
- City
- State
- Zip Code
- Telephone Number(s)
- Read the statement
- (Optional) If there will be additional instructions they may be added in the section provided – if more space is required, continue the additional instructions on a separate sheet and attach it to this document
- Principal must provide their signature
Step 3 – Witnesses Information – Each witness must enter their respective information as follows:
- Witnesses Names
- Street Addresses
- City, State, Zip Code
- Telephone Numbers
