Missouri Last Will and Testament Form

The Missouri last will and testament is a legal document that will guide an individual (Testator) through the personal process of creating a record of the desired distribution of their estate after death. The estate may include personal property, financial accounts and any personal property. The Testator may declare an executor within this document to distribute their estate to their chosen beneficiaries. The Testator must be of sound mind while completing this document. The form may be revoked or changed whenever the Testator may decide it would be necessary.

How to Write

Step 1 – The Testator may begin by downloading the document – Submit the Testator’s full name at the top of the page

  • Provide the name of the Testator
  • City
  • County
  • Read the title “Expenses and Taxes”

Step 2 – Appoint an Executor – Appoint an executor by entering the following:

  • Executor’s name
  • Address
  • County
  • State of residence

If the first selected executor is unavailable, an alternate may be established in advance:

  • Submit the alternate’s name
  • Address
  • County
  • State

Step 3 – Disposition of Testator’s Property –Enter all beneficiary information in the spaces provided:

  • Beneficiary’s name
  • Address
  • Relationship to testator
  • Submit the last four (4) digits of the beneficiary’s Social Security Number
  • The Testator must read the additionalparagraphs in this section
  • If additional instructions will be necessary, add a sheet and attach it to the form

Step 4 – Titled Sections –

  • Omissions
  • Bond
  • Discretionary Powers of Personal Representative (subsections A through K)
  • Contesting Beneficiary
  • Guardian Ad Litem Not Required
  • Gender
  • Assignment
  • Governing Law

Step 5 – Binding Agreement – The Testator must read and agree to the first paragraph and submit:

  • Testator’s name
  • Date of the document’s exectution in dd/mm/yyyy format
  • Testator’s name
  • Printed name
  • Date signature in dd/m/yy format
  • Individual’s (Testator’s) name

Step 6 – Witness Signatures and Notary Public – Signatories are required to sign before a notary public

Witnesses –

  • Witnesses must enter their signatures
  • Addresses

Testamentary Affidavit –

  • The notary public will acknowledge this document
  • The notary will submit the names the Testator and Witnesses
  • Testator will provide their signature
  • Witnesses will submit their respective signatures
